This activity has been designed to meet the educational needs of nurses and physicians involved in the treatment and care of pediatric patients who have been victims of sexual abuse/assault.
Statement of Need/Program Overview:
Clinicians evaluating patients who have experienced sexual assault often fail to recognize the importance of comprehensive medical treatment and evaluation of the patient, which is included in the medical forensic examination. They lack basic training in performing the medical forensic evaluation of the patient, and are unable to provide the services or the care necessary to prevent long term health sequelae in the aftermath of sexual violence. Current peer reviewed literature indicates that to provide the best outcomes for the patient in terms of their health and well-being, a provider needs specialized education and training in the care of this patient population. This online training will provide clinical information, along with the cultural and historical information necessary for completing a medical forensic evaluation on the American Indian/Alaska Native populations that Indian Health Services provide services to.
After completing this activity, the participant should be better able to:
- Describe critical cultural concepts to understand when working with tribal communities.
- Discuss the scope of violence against AI/AN people in the US.
- Discuss the role and duties of a pediatric SAE.
- Define Evidence Based Practice (EBP).
- Differentiate how children react to trauma
- Recognize the impact trauma informed treatment may have on a child who has experienced sexual abuse.
- Define and distinguish between types of child maltreatment.
- Discuss the co-occurrence of various types of child maltreatment.
- Define “minor.”
- Describe perpetrators of sexual violence.
- Describe typical growth and development across childhood
- Explain informed consent as it applies to childhood
- Discuss obtaining a history from sources other than the patient
- Define and discuss the misuses of sexual maturity rating scales in child pornography cases
- Identify assessment techniques and the identification of strangulation injury in children
- Examine the regulatory requirements when for caring for a pediatric patient who has been sexually assaulted

Carmen is an enrolled member of the Cheyenne River Sioux Tribe of South Dakota. She has worked toward ending violence against Native women professionally and as a volunteer since 1988. In 1988, she began working in the field as a Children’s Advocate in a shelter, at which she held various other positions over the years. During that time, she served as a co-chair for the South Dakota Coalition Against Domestic Violence and Sexual Assault and as a consultant for the Center for Offender Management, Mending the Sacred Hoop, National State Courts and Sacred Circle- National Resource Center to End Violence Against Native Women. Carmen currently serves as the coordinator for the Native Women’s Society of the Great Plains, a tribal coalition whose membership consists of Native programs, providing services to women who experience violence, across the northern Great Plains. The Native Women’s Society is in its third year of existence and has grown to 22 members strong. The Coalition supports its membership through training, technical assistance and promotes awareness to begin te process of keeping women safe in their own communities. Carmen’s areas of expertise include civil protection orders, children’s issues around domestic violence, and advocacy training for the areas of sexual assault and domestic violence. Other areas of service include: school board member, part time Tribal Magistrate, member of the local Indian Child Welfare Board, the local tribal Child Protection Team, founding member of the Okiciyapi Oti Habitat for Humanity, and current licensed lay advocate for the Cheyenne River Courts. Carmen takes pleasure in the occasional rodeo and pow wow, but is happiest at home with her children, family, friends, horses and cows. The changing seasons and openness of the Plains keeps her grounded and enjoying life.
John W. Sawney (Cherokee) has worked in criminal law for more than 13 years with experience both as a prosecutor and indigent defense attorney. He has successfully prosecuted domestic violence and sexual assault cases in Indian Country. Mr. Sawney served as the managing attorney for the District 27 Drug Task Force and Child Support Enforcement covering four counties in Oklahoma. He is a member of the Oklahoma Bar Association.
A 24-year veteran of the criminal justice field, Gayle Thom recently retired from the FBI during her 10th year as a victim specialist on the Rapid Deployment and Evidence Response Teams (ERT). In an agency wide vote, Thom was one of six Bureau personnel nationwide, tapped for the FBI American Indian/Alaskan Native Advisory Committee. Thom is a graduate of the Professional Certificate in Victim Assistance: Critical Analysis program, established by the Joint Center for Victim and Violence Studies, Washburn University. In October 2006, she received the highest award bestowed on FBI employees, The Director’s Award for Excellence, for her response to the tragic Red Lake Nation School shooting in Minnesota. She was honored with the National Crime Victims Rights Week Award in 2001 and in 2004 for outstanding dedication, service and contribution on behalf of crime victims in Indian Country. For seven years, Thom was assigned to the Rosebud and other Reservations in South Dakota, assisting victims of crime in Indian Country. Most recently she was assigned to the Pine Ridge Reservation, where she says she was humbled to receive the Oglala Lakota Nation Dedicated Service Honor in May 2004 and again in 2009. She was also nominated for the 2005 Service to America Medal and the Attorney General’s Award for Outstanding Contributions to Community Partnerships for Public Safety. Thom was appointed to the Tribal Victim Assistance Advisory Committee in 2007. She also serves on the Children’s Justice Act Advisory Committee, which wrote the Partnerships for Indian Communities Report in July 2008, for the Department of Justice Office for Victims of Crime, through the Tribal Law and Policy Institute. In 2007, Thom was selected to help implement and to chair the nine-member FBI Victim Specialist advisory group. She introduced the Victim Witness Assistance block of training into the South Dakota Law Enforcement Training Academy curriculum, serving as the adjunct instructor. As a member of the FBI ERT, she is a graduate of the National Transportation Safety Board Training Academy and has extensive additional training in crisis response. An FBI-certified trainer, she has lectured and trained throughout the U.S. and in Canada in a variety of criminal justice capacities.
