Children are not little adults. In the context of a sexual abuse or sexual assault evaluation we are talking with children about sensitive and highly emotionally charged issues. We often ask children questions that would be difficult for many adults to answer. Understanding child development is critical to a successful interview, which is even more important in the presence of trauma. It is the responsibility of the interviewer to assess the child’s developmental status and then ask questions accordingly.

The need for this session has been established through a review of peer‐reviewed literature and media coverage. The Adverse Childhood Experiences (ACE) Study is a long term, in‐depth, epidemiologic analysis of over 17,000 middle‐aged, middle‐class Americans, matching their current health status against 10 categories of adverse childhood experiences that occurred on average a half‐century earlier.
This ongoing collaborative effort by Kaiser Permanente and the CDC found that: adverse childhood experiences are surprisingly common although typically concealed, and are overwhelmingly unrecognized in medical practice; they still have a profound effect 50 years later, now transformed from life experience into organic disease and mental illness; and that adverse life experiences in childhood are the main determinant of the health and social well‐being of the nation.

Clinicians need further training regarding the six types of child sexual exploitation, recognizing that often more than one type of abuse exists in these types of cases. The role of the Internet and communication technology, particularly 3G and 4G technology allow for easier victimization of children and a less likely occurrence of apprehension and subsequent prosecution. Emerging research underscores that the victim impact is worse in child sexual exploitation than in child sexual abuse, primarily because of recurrent nature of the abuse and the likelihood that these represent a multi-offender case in many types of sexual exploitation.

The health impacts of intimate partner violence (IPV) are well documented in the scientific literature. There are a wide variety of comorbid health complaints described, ranging from chronic pain complaints to gynecological disorders to cardiac disease. Because of this, clinicians must approach the care of IPV patients as more than just the documentation of an acute episode of violence. All healthcare providers, regardless of specialty, need to understand how experiencing violence, including IPV, impact both the short- and long-term health of patients.
